This series contains the image of the Motor Vehicle Crash Report (SFN 2355), which is filled out by the officer when an accident occurs. This form has detailed information about the accident. Also contains Officers' Collision Diagram (SFN 14267), Crash Field Notes (SFN 14315), and Animal/Motor Vehicle Report (SFN 50979). This series also contains the Crash Reporting System (CRS) which is crash data submitted by law enforcement for statistical reports and electronic crash report generation.
